How do you keep a Traveler's Notebook setup from getting bulky?

I'm trying to figure out what would make a Traveler's Notebook practical to carry every day. It's tempting to give each kind of note its own insert, but then I'd be carrying things I might only use once in a while. I'd start with the inserts I reach for daily and leave occasional-use ones at home. How do you decide what gets a spot in your regular setup?

I group occasional notes in one catch-all insert. A subject gets its own only when I need to find it often or handle it differently; otherwise, that separate insert is extra bulk for something I rarely use.
